However, simply coming into contact with water containing the organism does not necessarily cause an infection. The greatest danger occurs when contaminated water enters the nose, allowing the amoeba to travel through the nasal passages toward the brain.


Once Naegleria fowleri reaches the brain, it can cause severe inflammation and extensive tissue damage. The resulting illness progresses very quickly and is almost always fatal. Early symptoms can sometimes resemble those of more common illnesses, making diagnosis especially difficult. As the infection becomes more severe, the patient may experience serious neurological symptoms as the brain becomes increasingly damaged. Because the disease is so rare, many doctors may never encounter a case during their careers, which adds to the challenges of identifying and treating the infection quickly enough.


Despite the frightening nickname “brain-eating amoeba,” infections caused by Naegleria fowleri are extremely rare. Health authorities have reported that the United States usually sees fewer than ten cases each year. Between 1937 and 2025, approximately 180 confirmed infections were recorded in the country. Although the number of cases is very small compared with the millions of people who participate in freshwater activities every year, the infection receives significant attention because of its extremely high fatality rate.


The Louisiana Department of Health emphasized that Naegleria fowleri occurs naturally in the environment. The amoeba is most commonly associated with warm freshwater and tends to thrive when water temperatures are high. It becomes more of a concern in water that remains above approximately 77 degrees Fahrenheit, or 25 degrees Celsius. For many years, infections were reported mainly during the summer months in the southern United States, where temperatures can remain high for extended periods.


However, scientists have also observed cases appearing in areas farther north. In recent years, infections or concerns related to Naegleria fowleri have been reported in states such as Maryland, Indiana, and Minnesota. These cases have contributed to discussions about changing environmental conditions and the possibility that warmer temperatures may allow the organism to survive in a wider geographic area. Nevertheless, infections remain exceptionally uncommon, and experts continue to study the environmental factors that influence where and when the amoeba may be present.


One of the major difficulties in preventing infection is that there is no simple, routine environmental test that can reliably determine whether Naegleria fowleri is present in a particular lake, river, or other natural body of water at any given time. The amoeba may occur naturally in some areas and may not be evenly distributed throughout the water. As a result, the absence of a detected organism at one location or time does not necessarily guarantee that a body of water is completely free from it.


Health officials have also explained that the infection cannot spread from one person to another. It is not contagious, and individuals cannot contract the disease through ordinary physical contact with an infected person. The infection occurs when water containing the amoeba enters the nose under circumstances that allow the organism to move toward the brain. Swallowing water is not considered the same risk because the amoeba must enter through the nasal passages to cause PAM.


The death of Lillian Smart has deeply affected Louisiana residents and has attracted national attention. Louisiana Governor Jeff Landry expressed his sadness publicly, writing that he was heartbroken by the news of Lillian’s passing. In the previous year, a 12-year-old boy in South Carolina also died after developing an infection caused by Naegleria fowleri. The story has encouraged renewed public discussion about freshwater safety, particularly during periods of extremely hot weather. Health experts generally advise people to be aware of potential risks when swimming or diving in warm freshwater, especially in locations where water temperatures are unusually high. Reducing the amount of water forced into the nose can help lower the already very small risk of infection.


At the same time, experts emphasize the importance of keeping the danger in perspective. Millions of people swim, boat, and participate in recreational activities in lakes and rivers every year without becoming infected. Naegleria fowleri infections remain extraordinarily rare. Nevertheless, the seriousness of the disease means that public awareness is important, particularly because the infection progresses rapidly and can have devastating consequences.


The loss of Lillian Smart is a heartbreaking reminder of the unpredictable dangers that can sometimes exist in the natural environment. For her parents, family members, friends, and community, statistics cannot measure the depth of such a loss.
